Making A Nutrition Department Appointment

We accept referrals from all members of your Atrius Health health care team.  You may be referred to our nutrition department by your Primary Care Provider, or any doctor, nurse practitioner, or physician assistant from other departments.

To schedule an appointment, call our Central Nutrition department at 617-657-6433. We are available Monday - Friday from 9:00am until 5:00pm. 

Many health insurance plans cover nutrition visits, however, we advise you to check with your plan to confirm the details of your coverage.

Before Your Visit

To prepare for your first appointment, please bring a list of, or any bottles of vitamins/supplements you are taking, and at least a 3-day record of all food/beverages and exercise. Click here for a printable food and exercise journal form.
