The Advanced Practice Clinician Collaborative (APCC) is an innovative program whereby a group of licensed advanced practice clinicians (nurse practitioners and physician assistants) work with physicians and experienced APCs to care for patients and further develop their clinical skills. As a patient in the APCC, you would schedule your visits with an APC on the team who provides your care.

Atrius Health is excited about this program as it allows patients to receive highly attentive care while these APCs work with and learn from our longtime providers. Currently the program is only offered at our Concord practice location.

Does my insurance cover the APCC program?

Yes, all the providers in this program are licensed Advanced Practice Clinicians (APCs) and credentialed with the insurance plans we accept at the practice.  

How do I sign up for this program?

Please contact our Central Registration department at 1-800-249-1767 and let them know you would like to join the APCC program at our Concord office. They will complete your new patient registration and provide additional information if your health plan requires notification of any PCP changes. After that, you can call the Concord practice directly to schedule future appointments with any APCC clinician. As our patient, you will have access to all available services at Atrius Health including on-site pharmacy, radiology/imaging, laboratory, 24/7 telephone medical advice and urgent care.

If I choose to participate in the program, will I see a physician?

APCs in the program work with physicians in the practice and your appointments will be with one of the APCs. If the APCs determine that your medical condition requires you to see a primary or specialty care physician, they will refer you. At times, APCs may consult with a physician, and you may also be visited in the exam room by precepting physicians or APCs involved in the program. Your medical record/history will be available to your Atrius Health treating care team members.

Will I always see the same APC in the APCC program?

If desired, we will try whenever possible to schedule you with the same APC when you need care; however, you may see multiple team members depending on your scheduling preferences and appointment availability. A goal of the APCC program model is to provide patients with enhanced appointment access through multiple team members.

Please note that after one year of working as part of the APCC, the APC clinicians will transition from the program and move to a different Atrius Health primary care office to care for patients. At that time, a new team of APCs will join the program in Concord and be available to care for you.

If I do not wish to continue in the APCC program, can I be transferred to another Atrius Health PCP?

Currently, there are not any adult PCPs accepting new patients at our Concord office. You can contact our Central Patient Registration department at 1-800-249-1767 to consider options for selecting a PCP at another Atrius Health primary care location welcoming patients. We encourage you to monitor our website for any changes in adult PCPs accepting patients at the Concord practice and other locations. We are always actively recruiting for additional physicians to join our staff and we hope to expand the Concord physician team.
