Anticoagulation Management Service
Specially-trained registered nurses and clinical pharmacists manage the safety and effectiveness of prescribed chronic anticoagulation with warfarin and direct oral anticoagulants (DOACs) for over 4,000 Atrius Health patients. We work together with Internal Medicine, Family Medicine and related specialties to provide the best possible care for our patients. The Atrius Health Anticoagulation Management Service (AMS) is recognized as a National Center of Excellence by the Anticoagulation Forum.
AMS services begin with a referral by your physician to our service after you have been prescribed an anticoagulant medication. Once you are enrolled in our AMS, our nurses and pharmacists will provide you with medication education, prescribing to meet your clinical goals, helping with your anticoagulation if you have a medical procedure or surgery, and ordering and interpreting all laboratory tests needed for your anticoagulation.
